5180:2-12-20 Cribs - Safe Environment and Practices

“During the inspection, it was determined that infants younger than twelve months old were placed in a position other than on their backs to sleep. The current version of the prescribed DCY 01235 Sleep Position Waiver for Child Care form which documents written permission for an alternative method signed by the childs physician, was not on file, as required by this rule. Discontinue this practice immediately until this waiver has been secured. Provide staff training. Submit the programs corrective action plan, which includes a copy of the prescribed form signed by both the childs physician and the parent, and a statement that training was provided, to the Department to verify compliance with the requirements of this rule.”

5180:2-12-14 Transportation and Field Trip Procedures

“In review of the program's records, it was determined that requirements for written permission from the parent/guardian for a field trip or routine trip were not met.The following was observed: Written parental permission was not secured for field trips and/or routine trips off the premises.Submit the program's corrective action plan to the Department to verify compliance with the requirements of this rule.”

“In review of the staff records, it was determined that background check requirements had not been followed, for the individual(s) listed on the Employee Record Chart.The following was observed: The DCY 01177 \"Individual Notification of Background Check Review for Child Care\" was on file instead of the DCY 01176.Submit the program's corrective action plan, which includes a statement that the correct form is now on file, to the Department to verify compliance with the requirements of this rule.”

“In review of the staff records, it was determined that the tuberculosis (TB) screening for 3 of 17 employees, including child care staff members, did not meet the requirement.The following was observed: A TB screening was not on file prior to the first day of employment.Submit the program's corrective action plan to the Department to verify compliance with the requirements of this rule.”

The JFS 01177 \"Individual Notification of Background Check Review for Child Care\" was on file instead of the JFS 01176.2. The JFS 01176 on file was for a different program.Submit the program's corrective action plan, which includes a statement that the correct form is now on file, to the Department to verify compliance with the requirements of this rule.”
