“LS observed one (1) teacher in Room 7 with eleven (11) children. LS inquired as to where the teacher is and was informed she left at 11:30am. During inspection, Aide was placed in Room 7 to bring child/staff ratio compliant. Corrected on site.”

“LS conducted complaint investigation on today's date. Complaint alleged a one (1) year old child came to daycare and the vaginal area was swollen and there was a red discharge. The Center stated they would wait and to see if it gets worse before reporting it. Center workers expressed their concerns and mentioned that nothing was wrong with the child on Monday. Center staff is required to report abuse and neglect to protective services.”
