The record for Child 3 did not contain documentation of health information as required by the standards. 4. The record for Child 4 did not contain documentation of health information as required by the standards. 5. The record for Child 5 did not contain documentation of health information as required by the standards. 6. Staff 2 stated that the required health information was maintained at the administrative office.”
