“At the time of the inspection, there was no evidence of a fire drill being completed during the month of October 2025. Please ensure that fire drills are completed each month, and disaster drills twice a year. The operator submitted a letter of correction during the inspection today.”

“There is one staff member who has left employment and the OCC did not receive written notification of this from the operator. The operator informed the specialist today and it was documented on the discrepancy and staff change form. In the future, please provide written notification to the OCC about the ending of employment of an employee or staff member within 5 days of its occurrence.”

“The emergency form for 1 child in care did not contain the home address .The parent completed the emergency form on site. In the future, please ensure that the parent completes, signs, and dates the emergency form.”

“The Licensing Specialist observed a bin of hazardous products which included baby shampoo, baby bubble bath, baby powder, and several tubes of diaper rash ointment on the shelf of the diapering station. The doors of the diapering station had a safety mechanism installed, but it was not being utilized. The bin of hazardous products were relocated and made inaccessible to the children in care during the inspection.”

“The Licensing Specialist observed a miniature plastic slide in the infant and toddler classroom. The infant and toddler teacher shared that the slide has been utilized indoors. Although the infant and toddler classroom has a carpet, all outdoor equipment shall be utilized in the fenced in area with grass to ensure that there is plenty of resilient surfacing. During the inspection the principal provided the Licensing Specialist with a written statement explaining that the slide will only be used outside.”

“The Licensing Specialist noted that the written individualized activity plans did not have a staff member assigned to each infant and toddler. The children in the infant and toddler classroom were assigned to a staff member during the inspection.”

“The Licensing Specialist noted that the written schedule of daily activities did not include outdoor play. Morning (am) outdoor play and afternoon (pm) outdoor play were added to the written schedule of daily activities during the inspection.”

“The OCC was not notified of staff member, HH's departure. The principal completed and submitted a 1203 for HH during the inspection.”
