There was one infant in a pack and play outside of the nap room. Provider is reminded that staff are to remain with in the room with sleeping children, and staff-child ratio is to be maintained at all times for children under two.”

“Licensing Specialist observed a two year old in a pack and play with a blanket. Provider is reminded a child who is over two must sleep on a cot, and a child in a pack and play cannot have a blanket. Child was moved to a cot at the time of the inspection.”

“Licensing Specialist observed a water temperature of 125.6 degree. Provider is asked to have water temperature turned down and let Licensing Specialist know so that a follow up can be conducted.”

“Providers stated that they place the 7 month old child to sleep on their stomach and that the child cannot roll over on their own. Licensing Specialists reminded providers that this is not safe sleep practices and recommend staff take a safe sleep course.”

“Only the director is qualified as a lead staff member, Provider and Aides state that Aides are typically left alone with infants or older children as primary care givers. Facility is asked to submit documents to qualify other staff members to ensure children are under qualified care. It was mentioned by staff that the ratio for infants is 1:4, Licensing Specialist reminded them that they ratio is 1:3 and whenever there are more than three children under the age of two there needs to be two staff and at least one qualified infant/toddler teacher.”

“Provider and Aides indicate that they started five months ago, they were not reported to the Office of Child Care until January 2024. There are several staff listed on the associated party list that are no long affiliated with the child care.”
