      “The provider was out of compliance with R430-90-11(1)(e) for not ensuring that each caregiver provided and maintained active supervision of each child. During the investigation, it was determined that a child walked away, unsupervised, from their child care group and was found by a member of the public. The caregiver stated that the child had not been accounted for for about 20 minutes therefore they did not know the number of children in their care at any time.”
