“LS observed all 4 staff members lined up against the wall in the shade while children ran throughout the playground, instead of being stationed throughout playground. During playground observation, a child had fallen off of the playground equipment, bumping their cheek. No staff member responded. The child ran over to a staff member for help. Please review appropriate playground supervision with all staff and submit evidence of review, along with a plan of action.”

“LS observed in grasshoppers room that there were 20 children present in classroom, but 17 children signed in on bright wheel app on the tablet and in the classroom binder. In bumblebees classroom, there were 17 children in the room and 16 children signed into classroom binder. LS asked staff person the procedure for sign in, to which they stated that if parents are unable to sign in, the staff sign the children in at nap time. Staff person signed the 3 children in once it was brought to their attention. Review child security with all staff and submit evidence of review and plan of action to LS.”

“LS observed in Caterpillars black marks near the base of the wall, beneath the light switch; drip marks above the wall of the cots, near the trash can, water cooler, and bathroom; noticeable crumbs on one section of the carpet; buildup near the magnetic strip at the bottom of the fridge. LS observed grime and build up on walls throughout the bumblebees classroom. LS took photographic evidence of spillage in listed classrooms. Please submit a plan of action, explaining procedure moving forward.”

“LS observed in multiple rooms throughout the building sink temperature exceeding 120 degrees F. Please reduce water temperature and submit a letter of corrections.”

“LS observed in caterpillars room an air temperature at ground level of 64.5 degrees F at naptime; ground level air temperature of 61.8 degrees F in grasshoppers classrooms. Both classrooms had children present and are controlled by the same thermostat, located in the caterpillars classrooms. Facility shall submit a plan of action to ensure proper maintenance of air temperature at or above 65 degrees F at ground level.”
