“Children under 10 years of age always shall be within actual sight and sound supervision of staff. On July 8 2026, the center was unable to locate Child #1(age 7 years) for approximately 10 to 12 minutes. The center participated in an off-site field trip and the location had multiple floors. Child #1 was with their group, which consisted of 3 staff members and 2 parent chaperones and 29 children, as they entered the 4th floor of the facility. The grouping gathered at approximately 2:20pm to conduct a name to face check of the participants and transition to a new floor. At that time, it was observed that Child #1 was not within the line. 2 staff stayed with the grouping, and 1 staff person did a sweep of the 4th floor and continued to the 3rd floor, and then the 2nd floor. Upon reaching the 2nd floor, a supervisor was notified that Child #1 was unaccounted for during the name to face check. The supervisor was able to locate Child #1, hiding within a bush, on the 4th floor approximately 5 to 7 feet from the grouping of children and center staff. Child #1 stated they were able to hear staff calling for them, but was overwhelmed and did not respond. A text alert to staff was sent at 2:32pm confirming that Child #1 was located.”
