“LS conducted a complaint inspection in which it was alleged that a child was bitten, scratched, and blood was drawn. The parents did not receive an injury report for the child's injuries but they did receive an incident report for their child biting someone else. At the time of the inspection, LS asked for the incident reports for that incident. There was no incident report on file for this particular day when they child was hit/scratched and blood was drawn.”
