“LS observed the two toy shelves that were pushed together. The back of the shelves were exposed. The back of one of the shelves had staples that were protruding from the back board. Director informed LS that normally the teacher has the toy shelves open so the back of the toy shelves are not exposed to the children. Corrected on site.”
