“The playground is very close to a body of water. The playground is encompassed by a fence. However, the gate to the fence is no longer working properly. During the inspection, the gate was not locked and when tested, it was intermittently self-closing and self-latching. It may have settled. Please adjust the gate so it is again consistently self-closing and self-latching. It is recommended that the gate be locked since it is so close to the body of water. For the correction of this non-compliance, the Center may send pictures and a video showing that the gate was repaired so that it is consistently self-closing and self-latching.”

“During a review of a sampling of children's records, it was found that one of the children in the After Care program is missing the results of his blood lead test. There was evidence that the physician ordered the test, but no results were in the file. During the inspection, the Director found the results in the nurse's office and then added the results to the child's file. Therefore, this non-compliance was corrected on-site.”

“During a review of staff files, no medical evaluation was found in the Center's file for one of their substitutes. When asked, that substitute reported that she had one done before she started. While still on-site, the substitute located her completed medical that is recorded on the OCC form and sent it to OCC. She will also put it in her Center file. Therefore, this non-compliance was corrected on-site.”
