“Based on a review of records and interview, the religious exempt program did not ensure that the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years were obtained for each employee. Evidence: Eight staff records were reviewed. The record for staff #4, whose employment began 8/17/2020, contained a Signed Sworn Disclosure Statement that indicated that staff #4 resided in Massachusetts within 5 years of her employment. The file for staff #4 did not contain the results of a child abuse and neglect registry or equivalent registry from Massachusetts. The office manager confirmed that this was an oversight and that this documentation had not been obtained for staff #4.”
