      “The provider was out of compliance with Rule R381-100-13(4) by not ensuring that choking hazards were inaccessible to children younger than three years old. In the 2's classroom, there was small plastic hippo, which was a choking hazard, in a drawer, accessible to children younger than three years old. On the 0-2 playground, there was a broken piece of plastic, which was a choking hazard, on the ground, accessible to children younger than three years old. This was a repeat rule noncompliance as noted on 9/21/2023 and 10/27/2025.”
