“Based on record review and interview with staff, the center did not ensure each person in a supervisory position must be certified by a practicing physician or physician assistant to be free from any disability that would prevent them from caring for children under their supervision. Evidence: Staff #4 confirmed that there was no documentation that Staff #3, who is in a supervisory position, has been certified by a practicing physician or physician assistant to be free from any disability that would prevent them from caring for children under their supervision.”

“Based on observation and interview with staff, the center did not ensure handwashing by staff and children before eating and after toileting / diapering. Evidence: Staff #4 confirmed they only use hand sanitizer on the children’s hands before eating. The had not implanted the hand washing procedures because the staff confirmed they only use hand sanitizer before eating.”

“Based on record review and licensee interviews, the licensee did not ensure to obtain a Search of the Central Registry finding within 30 days of employment or volunteer service. Evidence: The center director confirmed, there were not findings of the Virginia Search of the Central Registry available for review during the inspection for Staff #1 (date of hire 8-19-2019) and Staff #2 (date of hire 9-7-21).”
