“Licensing Specialist observed that a staff member was not following the facility's discipline policy. Facility will need to review the their discipline policy with all staff and submit an acknowledgement that it has been reviewed with all staff at the center.”

“Licensing Specialist observed video showing Director putting both of her hands on a child, using force to remove the child from the breakfast line, and continued to restrain the child until another adult intervened. Facility will need to submit a statement of understanding as it pertains to the child discipline.”

“Licensing Specialist observed a group of 24 children in the Cafeteria with only 1 staff member qualifying as an aide at the time of inspection. Licensing also observed 33 children in Child Care Room with only 2 staff members present at the time of inspection. Facility to submit a written statement of their understanding that a child care teacher shall be assigned to each group of children as needed to meet the requirements for group size and staffing (1 staff to 15 children).”

“Licensing Specialist observed a group of 27 children in gym supervised by two aides at the time of inspection. Licensing Specialist also observed group of 24 children in the cafeteria supervised by only one Aide at the time of inspection. During the inspection, the Director moved substitute to the gym to switch places with one of the aides until additional staff scheduled to arrive later in the afternoon. The correction made was only for the one group in the gym and not in the cafeteria. Facility to submit a written statement of understanding that qualified teachers need to be assigned to each group of children at all times. Facility to also submit a plan to properly staff on days that schools have early closings.”
