Evidence: On 06/15/2023 staff could not explain how child A (age 15 months) sustained an injury to the inside portion of their left thigh which was later determined by medical staff to be a burn. Staff observed the injury around 11:20 am during a diaper change after children returned to the infant classroom from outdoor play. Staff reported that they initially thought the injury was a bug bite, which was reported to the parent of child A via a phone call. The facility requested that the parent pick up the child from the facility. The parent subsequently took the child to an urgent care facility for diagnosis and treatment where the injury was determined to be a burn. The parent later inquired with administration as to how their child sustained the burn. Administration could not determine, but could only speculate that the injury could have potentially occurred on a hot surface on the playground such as a slide, pour in place surface, or from a tarp that the children were sitting on for a group activity. The reported temperature on 06/15/2023 was mostly sunny with a high of 83 degrees.”
