“One caregiver who had juvenile fingerprints completed turned 18 and has not completed adult fingerprints.”

470 IAC 3-4.5-4(4) All open windows, doors which are kept open for other than entering and leaving, ventilators, and other outside openings shall be protected against insects by securely fastened 16 mesh screening. Cracks shall be sealed and sealing shall be in place around pipes, plumbing, and ducts

“In room 107 the screen has a big hole/detached from the area that opens the window. Screen needs secrely-fastened.”

470 IAC 3-4.5-4(1) All interior surfaces, equipment, materials, furnishings, and objects with which children will come in contact shall be well maintained, in a clean and sanitary condition, and of nontoxic durable construction

“In room 122 outlet covers were missing on southwall, room 107 outlet covers missing on east and west walls which can be safety hazard.”

470 IAC 3-4.5-5(f) Storage. Containers and packages of food, cleaned and sanitized utensils, equipment, and single-service articles shall be stored at least six (6) inches above the floor in a clean, dry location in such a way that protects them from contamination, cleaning compounds, and toxic or hazardous materials. This does not apply to cased food packaged in waterproof containers

“In the kitchen there was 3 bags of chips and a container of chips not dated. In the freezer there was a bag of tator tots not adequately sealed or dated.”

IC 12-17.2-6-14(2)(c) The child care ministry must do the following: is a person against whom an allegation of child abuse or neglect has been substantiated under IC 31-33

“1 Disqualified caregiver (DS) working without a child abuse/neglect check on file with the division. Send consultant notification of consent form submission.”

“Hazardous items and items labeled Keep out of Reach of Children were found accessible to children in the following areas: Infant room had Lysol spray and disinfectant wipes under the sink behind a child deterrent. Rm 120 room had air freshner on a table in an unlocked closet and hand sanitizer in a low unlocked drawer. Rm 122 had hand lotion on the teachers desk.”
