“There are curtains attached to the ceiling with push pins hanging throughout the home that are in reach of children posing a choking and suffocation hazard. One curtain was removed, exposing floor-to-ceiling shelving filled with craft materials, recycled items, and toys. The shelving is bowing from the weight of the items. All curtains needs to removed and items behind the curtains need to be stored in manner that does not pose a safety hazard to the children.”

“A child came into the playroom and was standing between the front door and the screen door. The sliding door to the main room got closed. The provider and co-provider were not aware that the child was not in the main room with them. LS returned the child to the main room with the provider.”

Near the second door, unused fencing, the trashcan recycling the rain water, gardening tools such as shovels, rakes, are watering cans are stacked in a pile, accessible to children. An open bag of soil is resting on a flower pot. Toys are strewn about the edges of the sidewalk on top of potted plants and in the bushes.”

“The home did not pass today's fire inspection. The fire marshal is concerned about the large amount of combustible items in the bedrooms, general housekeeping inside and outside of the home, access to the kitchen, access to the heat pump and breaker box.”

“The kitchen is off limits to children but the provider provides lunch to the children. The countertops are not accessible for use in the kitchen. The dining room table is used for lunch prep. The table is covered with papers, kitchen appliances, and other items, making it inaccessible for children to use. The provider is using sheets and curtains to block items stored along the walls and windows. Items are stored up to the ceiling behind the curtains/sheets. Near the kitchen, a shelf is stacked with binders and items resting on top. A mini kitchen in the playroom has papers and containers stacked on top. Shelving throughout the home contains papers, toys, and materials stacked on top, creating a risk of items falling on the children.”
