Facility needs to submit copies of the Medical evaluation and Basic Health and Safety training certificate to Licensing Specialist and a letter of corrections that they have been added to the staff member's file. Facility also needs to submit a letter of corrections that CBC results have been obtained and are available on site for the two staff members who were missing them at the time of inspection.”
