“The LS observed that B.M's parents gave the Provider two different Med. Admin Forms. One has written on the top not to give B.M. medication with the Parent's signature. Per the Provider, the parents have never given her medication. She will contact the parents and receive clarification about the forms. The LS spoke to B.M's father who confirmed that at this time his son only takes the medication at home. The parent will send a confirmation text to Provider.”
