“Provider did not send LS renewal paperwork prior to the inspection and the date requested on the renewal letter. During inspection provider stated to LS that she sent paperwork and LS should have received it by now. LS stated to provider she would check the mailbox once back at the office and let her know either way. Once back at the office LS did not receive the paperwork and informed the provider with an email.”
