AAC R9-3-301.A.4.d. A. A certificate holder shall: 4. Ensure that the provider: d. Prepares a dated attendance record for each day and ensures that each staff member records on the attendance record the staff member’s start time and end time of providing child care services for the child care group home;

“Findings include: Based on the Compliance Officer's observation and review of October's staff attendance records, it was determined that Staff #1 and Staff #2 were not documenting their start times and end times for providing child care services for the child care group home.”

AAC R9-3-202.D. D. A certificate holder shall ensure that each individual who is a staff member or an adult resident submits to the certificate holder a copy of the individual’s a valid fingerprint clearance card each time the fingerprint clearance card is issued or renewed every six years.

“Findings include: The Compliance Officer reviewed 2 staff files and found that Staff #2's (start date 9/1/2023) fingerprint clearance card had expired on 10/4/2023. Staff #2's fingerprint clearance card renewal application was received by DPS on 10/27/2023.”

AAC R9-3-401.B.2. B. A certificate holder shall ensure that: 2. The areas of the child care group home approved for providing child care services are maintained free from hazards;

“Findings include: Based on the Compliance Officer's observation, there were two cords dangling down from the wall pass-through window, accessible to the children.”

AAC R9-3-505.D.1. D. A certificate holder shall ensure that: 1. A stairway that leads to a floor or room outside of the areas of the child care group home approved for providing child care services is separated from the areas of the child care group home approved for providing child care services by either a door or gate that is kept closed during hours of operation;

“Findings include: Based on the Compliance Officers' observation, the stairway lacked a gate to separate the areas of the childcare group home approved for providing childcare services from the upstairs floors.”
