“The provider was out of compliance with R381-100-13(16) for failing to ensure that each accessible area to a child is free of any heavy or unstable objects. During the investigation inspection the evidence substantiated that a child was able to pull a bookcase down onto themself.”

R381-100-11(1)(a)-(g) Provide and maintain active supervision

“The provider was out of compliance with R381-100-11(1)(f,g) for not ensuring each caregiver provides and maintains active supervision of each child by positioning themselves so each child in their assigned group is actively supervised and by not remaining aware of the entire group of children even when interacting with a smaller group or an individual child. During the investigation inspection, the evidence substantiated that a caregiver was not providing active supervision. During this time, two children were playing in an unsafe manner and one of the children was injured. ”

R380-600-7(16)(a)-(e) Critical incidents reporting requirements

“The provider was out of compliance with R380-600-7(16)(a) for not submitting the report of a critical incident to OL in a format required by OL within one business day of the critical incident occurrence. During the investigation, the licensor identified two critical incidents of a child who left staff's supervision, undetected and was not reported to the OL.”
