“Medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Medications were being kept locked at the center but the backpack used during field trips for medications was not being kept locked.”

“Prescription medications must be maintained in the original, labeled container and the center is required to have obtained written authorization from a parent or guardian. One prescription medication for child #4 was at the center not in the original, labeled container. There was an authorization form completed for child #5 but it lacked the required information of parent signature.”

“Staff are required to at least every two years from the date of the first initial screening or testing, shall obtain and submit the results of a follow-up tuberculosis screening. Staff #1 was due for a repeat TB screening about 2 weeks ago.”
