“Based on record review, and interview, the center failed to obtain a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years. Evidence: 1. The record for staff #7 indicated that they lived in another state in the preceding five years. 2. The record did not contain a search of the child abuse and neglect registry from that state. 3. Administration confirmed that the record did not contain a search of the child abuse and neglect registry from that state.”
