“Rm 232 didn't have its attendance updated to reflect the change in the group after 12 noon. Some children go home at noon and some go to nap time a little after that. The room needs to be sure to keep its attendance procedure up to date to be sure not to lose anyone during the transition times. This was corrected during the inspection.”

“The center takes staff exemptions. All classroom staff need to complete the annual basic health and safety training update. There is 1 staff who needs the 2024 update and 1 staff who needs the 2025 update. This needs to be completed ASAP and certificates submitted to specialist.”

“A child was left unattended on the playground for approximately 24 minutes. The center did not report the incident to child protective services. Center is reminded that a child being left unattended falls under the definition of neglect and needs to be reported. Center needs to submit a statement of understanding their reporting requirements.”

“Program has added before and after care. There is care from 7-9am and 3-5pm. Center states it varies but approximately 10 children participate in those hours. The children come from the preschool program and their kindergarten program. The center stated that the staff with the children are the same staff as the OCC has cleared. Specialist observed that the before/after care is operated out of a classroom that is already approved by OCC. Children were observed being supervised by staff already known to OCC. The center states that the church started this at the end of the last school year. They didn't realize that before and after care has to be licensed child care. OCC will look into the staff qualification requirements needed for the before and after care staff as the preschool takes church program exemptions. Center needs to submit the Changes in Facilities form.”

“Complaint states that on October 1 it was reported that a 3 year old child was left unattended on the playground for 20-25 minutes in the afternoon between 2-3pm. Findings: Interviews with staff determined that a non-verbal child was alone on the fenced playground from approximately 2:30-2:50pm. Staff person 1 and Staff person 2 were outside with a group of 7 children. They came inside around 2:30pm. At that time staff person 1 had 3 of the 7 children come in with her and that left staff person 2 with the other 4 children. Staff person 1 took the children she had to their classrooms. She had to change one child and when she was done with that she left for the day. Staff person 2 came in with the rest of the of the children. She stated she did not do a count but thought she had all the children. Staff person 2 stated that it was noticed that the child was missing while preparing for snack. Staff person 3 was in the napping room. She came out of the napping room around 2:30 and sent the nappers to their respective classrooms. She went to the 2 year old room to work for the afternoon. See Remarks for further information.”

“There was a child in care who is the daughter of a staff person. The child is school age and the center is not approved for school age children. It was explained that she was there because her school is off this week and the mother doesn't have care for her. It was also explained that the child is there whenever her school is closed and her mother has to work. Center may not have children present who are not enrolled and may not have children present in an age group that the center is not licensed for.”

“The emergency card for the child who was left on the playground was reviewed. It needs an update and an emergency contact person added.”
13A.16.03.03B(1)
“One staff person stated that the attendance clipboards do go outside with them. Another staff person stated that no attendance goes outside with the group. All staff stated that they count the children. The fact that the child was left on the playground indicates that the staff did not have a good procedure for ascertaining whereabouts of all the children. Name to a face as children come into or exit the building from the playground is the best means of making sure everyone is present. Center needs to establish better procedures to ensure that children are not left behind.”
13A.16.03.06A(2)
“The center notified the specialist on 1/13/25 that two staff left employment on 11/30/24. Center is reminded that specialist needs to be notified of staff leaving employment within 5 days of the occurrence.”
