“At the time of inspection, provider had a resident living in the home who had turned 18. LS was made aware of this potential resident in Spring of 2024, and received criminal background checks, a current medical report, out of state background clearances as the resident is from out of state, and a signed privacy rights form. LS did not receive a notarized release of information, but after further communication with the provider, believed the resident to no longer be coming to live in the provider's home. LS did observe the resident in the home at the time of inspection. As the resident is now living in the home, LS requires a notarized release of information to be submitted as soon as possible to OCC.”
