“During the inspection, it was determined that equipment, materials and furnishings provided for indoor and outdoor play did not meet the requirement of the rule. The following was observed: The equipment was not yet in the new classroom, so it was not able to be observed. Submit the program's corrective action plan to the department to verify compliance with the requirements of the rule.”

“During the inspection, it was determined that emergency phone numbers were not posted in a visible location readily accessible to staff in each space used by the children. Submit the programs corrective action plan to the department to verify compliance with the requirements of this rule.”

“During the inspection, the preschool special education program did not meet requirements of section 3323.022 of the Revised Code and rule 3301-51-11 of the Administrative Code. Room 104 AM was identified as an integrated classroom; however, the current AM enrollment is 9 children that includes 6 IEP students and 3 typical peers. Classroom does not meet the 50% or fewer children with disabilities requirement.”
