During incident investigation, rep obtained a written statement from the staff who was present during the incident. The written statement confirms that although the staff was actively supervising his group of children on the playground, the staff also allowed children of all ages to utilize the slide. The manufacturer's suggested age range for this slide is 2-6 years of age.”
