EVIDENCE: Licensing Inspector review 6 records. California Sex Offender Background Check for Staff #4 was not completed before employment; New York Sex Offender Background Check for Staff #5 was not completed before employment indicated. Both records did not document the date of employment. Both staff members were observed working during the inspection.”
