“The provider self reported that excessive force was used by two staff members with children. During an investigation, it was determined that two staff members used excessive force with children. Resolution: Both staff members were given formal counseling forms which explain the corrective actions required. One staff member resigned and the other staff member has been suspended. In addition to suspension, a monitoring period of four weeks of classroom observation and supervision is included.”
