“The provider and child were walking to the provider's van so they could take other children to school and the child tripped. Because of her diagnosis of Persistent Concussive Syndrome, the mother, as a precaution, took the child to urgent care once she picked her up that afternoon. The provider wrote up an incident report but did not contact this office. The incident report was shared with the Licensing Specialist during the inspection. The provider is reminded of the regulation and will from this point forward contact Office of Child Care if a child is seen by a medical professional after an incident in her care.”
