If a child is over the age of two, only one lead blood test is required to be completed after the child turned two. Please have the parents obtain the lead blood test documentation from the physician and add to the children's files.”

“All of the enrolled children's files were reviewed. There were 12 emergency cards missing the doctor information, 9 missing the required annual update and 1 missing from the file. Two children were missing part I of the health inventory which includes the individual needs portion completed by the parent. One child was also missing part II of the health inventory that is completed by the physician. There were 6 children who were either missing documentation of immunizations or updated, current immunizations. Please have the parents obtain the completed documents and add to the children's files.”
