The staff health report for each staff member should have been obtained prior to the staff beginning employment.”

“Based on a review of ten staff records and interview, the center did not ensure four staff had the satisfactory results of the fingerprint based national criminal background check prior to employment and within the required timeframe. Evidence: 1) The fingerprint-based national criminal background check in the record of Staff #2, employed on 09/02/21, was completed on 09/15/21. 2) The fingerprint-based national criminal background check in the record of Staff #4, employed on 09/13/21, was completed on 10/08/21. 3) The fingerprint-based national criminal background check in the record of Staff #5, employed on 08/26/21, was completed on 03/29/21. The facility must not accept a required background check that is dated more than 90 days prior to employment. 5) The fingerprint-based national criminal background check in the record of Staff #10, employed on 07/22/21, was completed on 10/13/20. The facility must not accept a required background check that is dated more than 90 days prior to employment. 6) During interview, a member of management acknowledged the required background checks were not obtained within the required timeframes.”

The sworn statement was completed on 09/15/21. 3) The central registry check in the record of Staff #4, employed on 09/13/21, was completed on 10/22/21. 4) The central registry check in the record of Staff #5, employed on 08/26/21, was completed on 03/06/23. 5) The central registry check in the record of Staff #6, employed on 09/01/22, was completed on 10/22/22. 6) The central registry check in the record of Staff #7, employed on 09/01/22, was completed on 03/16/23. 7) The central registry check in the record of Staff #10, employed on 07/22/21, was completed on 04/21/22. 8) During interview, a member of management acknowledged the required background checks were not obtained within the required timeframes. A complete sworn statement or affirmation is required before employment. A search of the central registry check is required within 30 days of employment.”
