“The provider did not have fire drills documented in July, September and October. She stated that she did the fire drills but did not write them down. The provider did not also document disaster drills. She must do at least 2 disaster drills per year and none have been documented this year. Please ensure the drills are documented as required.”

“There are 6 children that do not have documentation of their lead blood test in their file. Please have their parents obtain the documentation from the child's physician and add to their files. Children under the age of six must have lead blood tests done at 12 and 24 months. If the child is over the age of two, he/she must have documentation of the lead blood test done after 24 months.”

One child needs part II of the health inventory and another needs the entire health inventory completed in her file.
