The center did not ensure that a child received adequate supervision at all times from individuals assigned to provide care.”

On 1 day, a child exited the classroom and was alone in the hallway without supervision. On another day, A child exited the facility after a door was propped open while contractors entered and exited the building. The center did not ensure that a child received adequate supervision at all times from individuals assigned to provide care.”

“On 2 separate occurrences, the center was aware that a child was potentially subjected to neglect. On the first occurrence, a child exited the classroom and was alone in the hallway without supervision. This incident was not reported to the OCC or local CPS. On the 2nd occurrence, the same child exited the facility after a door was propped open while contractors entered and exited the building. The center submitted a notification to the OCC on July 8 2026 at 2:26pm. As of 8am on July 9, 2026 the OCC did not receive a notification from CPS regarding this case. The OCC submitted this incident to Baltimore City CPS on July 9, 2026 at 9am. The center stated that they called CPS and was informed it was already submitted.”
