“On 12/1/21, at approximately 9:25 am, staff person #1 answered the front door of the facility, leaving 3 children unsupervised in the main room, a combined group of 2 young toddlers and 1 older toddler. When asked if they were the only staff person at the facility, staff person #1 stated that staff person #2 was in the bathroom. A minute or two later, child #3, an older toddler, was observed in the back of the main room, where they could not be supervised by staff person #1, who was in the front room, as there was furniture partially obstructing the doorway and blocking staff person #1's ability to see, direct and assess the child. Staff person #2 was still in the bathroom and when they entered the main room and was asked who was supervising child #3, staff person #2 shrugged their shoulders. At approximately 10:20 am, all 3 staff were in the front of the main room when staff person #2 told child #3 to throw away their mask. Instead of using the garbage can in the front of the room, the child walked through the back of the main room and into the hallway alone and was heading towards the bathroom, until this certification representative informed all 3 staff that he had left the room. Before nap time, staff person #2 took child #4 to the bathroom to change the child's diaper. A few minutes later, child #4 was observed standing alone in the back of the main room and staff person #2 was still in the bathroom. Staff person #1 was in the front of the main room and neither staff could see, hear, direct or assess the child. At approximately 1:07 pm, staff person #1 and #2 were in the front half of the room with 1 child and staff person #3, the operator, was in the rear half of the room with child #2, #3 and #4. Staff person #3 walked to the front of the room and could not properly supervise child #3, as he was sleeping behind a table at the very back at the room. At approximately 1:55 pm, staff person #1 carried a child up to the front of the main room and could not properly supervise child #3 as he was sleeping behind a table at the very back at the room. On 12-3-21 at approximately 2:00pm it was observed that Staff person #3 answered the front door of the facility holding a young toddler in their arms. It was observed that Staff person #3 left 5 children alone in the childcare space to answer the door. Staff person #3 identified one of the 5 children left alone in the childcare space as a related child.”
