“Upon arrival LS observed a 6 year old child playing outside while the provider was supervising from inside the home. The LS asked the provider if she has written permission from the parent for the child to do this. The provider informed LS that she does not have written permission. Provider will forward LS documentation of written permission from the parents for the child to go outside while the provider supervises from the window.”

“Specialist observed no evidence of liquid fragrance free soap in the first aid kit as required. See list of required items under Resource Documents on the OCC website. Provider shall obtain fragrance free liquid soap for the first aid kit and submit photographic evidence.”

“Specialist observed that there is no evidence of an appropriate lead screening for one child and lead forms for six children were not updated at 12 and/or 24 months as required. Specialist observed no evidence of immunization records for two children in care or evidence of appropriate exception or objection as required. Provider shall obtain missing forms and updated forms and submit copies.”
