“Provider has two children over the age of 2 with suspected special needs who are napping in a pack and play. Provider needs to provide evidence that they need to remain in the pack and play from the children's doctors or have them nap on a cot or mat.”

“LS observed sanitizing wipes in the living room and various medications and other items for the dog in the dining room. These items were in reach of children. Provider moved the items at the time of the visit. LS discussed with provider keeping these out of reach of children at all times.”

“Several children in pack and plays had soft items in the crib, LS discussed with provider that there should not be anything in the crib with the children. Provider removed these at the time of the visit.”

“At the time of the visit, provider had two 2 year olds, two 3 year olds and one 4 year old in pack and plays for rest. Provider had other napping equipment in the basement and brought it up at the time of the visit. LS discussed with provider that the 2, 3 and 4 year olds need to be resting on the sleeping bags she has. Provider took the children out of the pack and plays at the time of the visit.”

“Provider needs to install a gate at the top of the stairs on the second level since preschool children rest in these bedrooms. Provider also needs to make sure a gate is used while children are in care at the bottom of the stairs. Provider will send a statement of correction and show photos that these are in place.”
