Evidence: The record for staff 4 documents the first date of employment as 2/29/24. The sworn statement for staff 4 documents staff 4 lived in another state within the last five years. The is no documentation in the file of a search of central registry or sex offender registry for the other state.”

Evidence: The record for staff 5 documents the first day of employment as 1/23/24. The central registry background check is dated 3/20/24.”

“Based on a review of the staff handbook and interview, the center failed to implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining cushioning material under playground equipment. Evidence: The cushioning material under the swings was not maintained resulting in a lack of cushioning material under the swings.”

“Based on interview, the center failed to implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards. Evidence: 1. The director stated the centers procedure is to ensure hazardous substances are kept locked. This is to include disinfectant wipes. 2. In the P1 classroom Clorox wipes were on the counter beside the sink. 3. In the P2 classroom disinfectant wipes were in a clear unlocked drawer in the bathroom used by children.”
