On the two's playground, there were16 screws on the fencing that extended from 6 threads to 18 threads long that created scratching and impalement hazards. There were leaves and debris observed underneath the Step2 slide/climber that created a potential for animal/reptiles. There was an unstable umbrella on a stand that created a crush hazard. 5. On the PreK/School Age playground, the helicopter climber had two loose bolts that created an entanglement hazard. There were 8 screws on the fencing that extended from 9 to 18 threads long that created a scratching or entanglement hazard. There was a mesh bag that was tangled and tied to the fencing that created and entanglement hazard.
