This complaint is substantiated for not following the child health observation procedures because Provider #1 stated she observed the scratch over Child #1's eye upon arrival at the family child care home but did not document or inform the parent. Provider #1 stated she assumed Child #1's mother was aware of the scratch since Child #1 had long fingernails. Provider #1 stated she also observed Child #1 s puffy eyes upon arrival.”

An additional concern noted was [name] began using an unapproved space for child care without the approval of OCCL and the fire marshal.”
