The total # of children in class was 11. Child #1 was left in the classroom asleep. Once the children were loaded on the bus, staff #3 did not take the role. Staff #4 did a sweep of the classroom and found child #1. Child #1 was left out of sight and sound supervision for approximately 13 minutes.”
