“The center reported that a staff person, Ms. B, smacked two children on the hand and pushed one of those children causing the child to trip and hit a trash can. This happened on 8/6/2026 during outdoor waterplay. It was reported that one of the hand smacks was because the child was licking his runny nose. Written statements were provided. An aide stated that Ms. B pushed the child as he was asking for her help with his shoe, then he fell and knocked the lid off the trashcan. She heard a pop and then another pop when Ms. B popped the child for picking his nose and then eating it. When another child took the tablet from a staff person, Ms. B was observed popping another child. [name] written statement says she swatted the child's hand because he bites other children and takes other children's food everyday. She said the child does not listen when being told to stop and she does not get any help from the aide in her classroom because she is always busy on her phone. The director said Ms. B admitted to hitting and pushing the child and demonstrated the hitting of the child's hand. The center terminated [name] employment effective August 7, 2026.”

“The complaint alleges infants are sleeping on the floor and not being moved to the cribs after falling asleep because the staff person is unable to pick the infants up and move them to the cribs. It was reported that an infant was asleep on the floor on its belly for 30 minutes and another infant fell asleep upright in a Bumbo seat. Three times during the first 3 hours of the inspection, infants were observed sleeping on the floor (on Boppy pillows and in chairs). The first time it was stated the child just fell asleep and assistance was requested by the staff person to place the infant in her crib. Around 12:00 PM, two infants were observed asleep in unapproved sleep sources. One child was in an almost sitting position but asleep with the infant's neck bent back. This was brought to the director's attention and as it was the staff person called for assistance with moving the infants to the cribs. The staff person is also unable to lift the infants from their crib to take them out.”

“At the time of the inspection, there were 12 children in the two year old classroom (9 twos, 1 three, 2 toddlers). The group size is allowable but there were only two staff persons present. “The complaint alleges a staff person is picking up and moving infants with one hand while using a cane with the other hand to support herself. It was reported the staff person has limited mobility and sits in a chair above the children then leans over to tend to the infants. There are concerns if the staff person falls over onto the infants or how the staff person can safely the infants in an emergency. At the time of the inspection, the staff member was alone in Infant A with 3 infants. She was observed using the shelf to support herself and the cane for support when moving around the room. The staff person uses a chair in the room to tend to the infants who are on the floor. In order to feed one infant a bottle, the staff person moved the chair near the infant then was able to use both arms to lift the infant to her lap. One infant was crying for a bottle but the staff person was switching cribs for an infant that can roll over and she needed the crib for support during the task. It was reported that she calls for assistance with diapering and moving infants in and out of cribs; however, this did not appear to be happening each time.”
