“During the inspection LS observed 3 children missing a medication authorization form and observed at least 5 children's form that did not have a date of when the medication was received by the center. Facility is reminded that they must have a medication authorization form for each medication needed for a child as well as complete the bottom portion of the form when the medication is received from the parent or guardian. Facility is asked to submit a letter of corrections once the information has been obtained.”

“During the inspection LS observed at least 3 classrooms with piles that were about a foot and a half tall of miscellaneous items such as electric cookware, food in plastic containers, and paint on the classroom counters. Facility is asked to remove the items from the countertop, as they can potential fall on the child and submit photographic evidence upon completion.”
