Evidence: 1. Caregiver # 1's first day of employment was 2/27/23. The provider confirmed with Caregiver # 1 that she resided in Illinois within the last five years. 2. The provider did not complete a sex offender search and criminal record check for Caregiver # 1 prior to the first day of employment. 3. The provider did not submit a central registry search for Caregiver # 1 for Illinois.”

“Based on observation, the total number of children receiving care at any one time exceeded the capacity of the home. Evidence: 1. At 10:45 am, there were 14 children present in the direct care of 3 staff members. 2. Staff # 1 stated that Child # 1 and Child # 2 were not enrolled children, and were present only for a short period of time on this day. 3. Child # 1 and Child # 2 both left at approximately 11:15 am.”
