“The center shall establish and implement procedures for a daily simple health screening and exclusion of sick children by a person trained to perform such screenings. The administrator stated that staff had not received training in procedures for a daily health screening and exclusion of sick children. There were no staff at the center that were formally trained in daily health screenings at the time of inspection.”

“The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The record of staff #5, employed for two years and 11 months, did not have documentation of an out-of-state criminal history and child abuse and neglect background check being requested.”

“Upon enrollment of a child in a child day program, such child day program shall require information from the person enrolling the child regarding previous child day care and schools attended by the child. The records of child #1, child #2, child #3, child #4, child #5, child #6, child #7, and child #8, all present at the time of the inspection, did not have documentation of previous child day care and schools attended.”
